Medical Forms Policy
✅ What We Complete:
- School enrollment forms
- Camp & sports physical forms
- Immunization records for school/daycare
- Simple work excuse notes (up to 48 hours)
❌ What We Don't Complete:
- FMLA (Family Medical Leave Act) forms
- Disability documentation
- Insurance appeals & prior authorizations
- Complex administrative paperwork

Additional Services
Home Visits*
Essential Care members: $125 per visit
Comprehensive Care members: $99 per visit (beyond included visits)

Circumcision
In-clinic circumcision: $300–$450
In-home circumcision: $450–$600
Extended Care members receive this service included or significantly discounted.
Medical Ear Piercing
Safe, medical-grade ear piercing using the Blomdahl system. Medical-grade titanium earrings and sterile single-use equipment.
Both ears: $130 (non-members) / $99 (members)
Single ear: $75 (non-members) / $60 (members)
Extended Travel Radius
Most home visits are within 15 miles of the clinic. For visits between 15 and 25 miles, a travel fee of $40 may apply.
*All home visits and circumcisions must meet medical appropriateness criteria. Circumcision procedures are performed up to 60 days of age only and require prior vitamin K administration.
Frequently Asked Questions
Is this membership insurance?
No. Whole Pediatrics Group is a direct primary care practice. Membership covers services we provide in the clinic,
at home, and via telemedicine, but it is not a substitute for health insurance.
Can we use our insurance for labs, imaging, or specialists?
Labs and imaging can be ordered directly through me at significantly discounted rates, often better than insurance
copays. For specialist referrals, you would typically need to go through your insurance or primary care network.
Families are encouraged to keep insurance for medications, hospital care, and specialist visits.
